I acknowledge that my and/or my child’s presence at, preparation for, or participation in any event, show, rehearsal, audition, class, or workshop (“Event(s)”) offered by Ministry in Motion Inc. (“Ministry in Motion”) is entirely voluntary. I understand that these activities involve inherent risks, hazards, and dangers, including the risk of injury or death.
I voluntarily accept all risks on behalf of myself and/or my child and agree to participate only within our abilities. In consideration for being permitted to attend or participate in the Event(s), I agree to hold harmless and release Ministry in Motion Inc., Cross Culture Church, Celestial Dance, its owners, board members, officials, representatives, employees, contractors, and assigns from any and all liability for injury, damage, or loss to myself, my child, or our property arising from attending, preparing for, or participating in the Event(s).
I agree not to take unreasonable risks, including but not limited to the consumption of alcohol or drugs prior to or during participation.
I agree to comply with all written, verbal, and customary rules and conditions of participation.
I grant to Ministry in Motion Inc., its representatives, licensees, designees, permittees, and assigns the irrevocable right to film, photograph, videotape, and/or record myself and/or my child in any form of media.
I understand and agree that all recordings (“Products”) may be used in perpetuity, for any purpose, including but not limited to promotion, advertising, marketing, or distribution, in any medium now known or later developed, without further compensation.I authorize Ministry in Motion Inc. to enroll myself and/or my child in its text and email database and to send relevant communications.
I understand I may opt out at any time.
I understand that Ministry in Motion Inc. may substitute speakers, instructors, or choreographers when necessary due to scheduling conflicts or unforeseen circumstances.
I understand that my electronic acceptance or signature below is legally binding and considered equivalent to an original handwritten signature.
If requested, I agree to provide a physical signature on a printed version of this agreement.I understand that all purchases and reservations are final and that no cancellations or refunds are available for any reason.
By signing below, I acknowledge that I have read, understand, and agree to the terms of this Waiver and Release Agreement, and that I am signing voluntarily. I confirm that I have had adequate opportunity to ask questions.
